Community Offer
Local Coordination service is now live!
Adult Social Care have partnered with the British Red Cross to provide a new service which will promote health and well being, reduce isolation, help people connect with their community and promote independence, ultimately preventing, delaying or reducing their need for formal care
The British Red Cross Local Coordinators will be embedded with their locality area, working closely with Adult Social Care and the Voluntary and Community Sector. Local Coordinators will get to know an individual before supporting them and will co-design a plan with them. They will have an in depth knowledge of local opportunities and networks and play a key role in helping people to identify and access this support, build their resilience and increase their independence. They will also play a crucial role in identifying gaps in the community offer.

Adult Social Care is changing the way we offer support and advice and our new operating model will be delivered through Your Life, Your Way. The programme in adult social care is designed to:
- drive the best outcomes for residents
- ensure that the right support is delivered at the right time
- help to reduce the growing financial pressures across Adult Social Care
- enable us to manage demand within the system.
To support this shift we will support people to keep as independent as possible, and to be in control of their care and support, we have developed a new adult social care website – Your Life, Your Way – to provide early information, advice and access to services and support.

The new website embodies strengths-based approaches, reinforces our prevention and early intervention offer and meets our duties under the Care Act 2014.
We have also made the website user friendly, easy to navigate and it promotes better understanding of the options available to our residents.
It will also provide information and self-help tools that residents regularly ask our front-line and customer service teams for, in time reducing the number of contacts regarding the information that will be provided on the website and freeing up time for teams to focus elsewhere.
The website is in what’s called ‘Beta’ phase at the moment – this means it is live so we can test it and gather feedback from staff and residents to improve it further.
We will be launching the new website in April, adding in some final features and functionality during March. This will include more online tools that residents can use, such as a financial calculator and a community directory of services.
