Proportionate Reviewing Pilot
We are in the process of starting a project designed to pilot a new approach to how we undertake reviews across the service.
The aim of the proportionate reviewing approach is to ensure that people are receiving appropriate and proportionate care and support, as well as being enabled to meet their outcomes and maximise their independence. Through trialing this new approach we want to:
- Ensure reviews are targeted to deliver as many reviews as possible, but these will be targeted towards those individuals that will benefit most from the review
- Ensure the appropriate care and support has been identified to support people to meet their objectives with proportionality and appropriateness
- Develop a proportionate approach to reviews based on the need of the person and their families – a range of approaches will be on offer to the reviewer e.g. virtual, phone, face to face, provider led or partner led
We will be delivering a Maximising Independence intervention (training and huddles) to the Reviewing Team to support them during the pilot. There may be a need to bring additional resource into the reviewing project and we will keep you updated as this progresses.
The pilot project will run between October 20 and March 21. We will then evaluate the pilot and this will inform the approach going forward.

Change Champions
The YLYW change programme recruited change champions to support with the maximising independence workstream back in August. Change Champions have been shadowing various interventions delivered on the workstream and will now be co-delivering alongside the project team. Hi I am a Social Worker in the Holderness Team and I am one of the change champions working on the maximising independence work stream of the YLYW programme. I wanted to be a change champion after participating in the initial pilot for the strengths based huddles and experiencing the positive outcomes that reflective practice can foster. I also found the behavioural science behind 'mindspace' to be an interesting approach when working towards change and improving strengths based practice.
Hello, I am a social worker within the Enhanced Hospital Team for Goole Community Wellbeing Team. For me, being a Change Champion enables me to be continually reflexive in my own practice while supporting others with engaging and understanding strengths based practice through the maximising independence workstream for the 'Your Life, Your Way' programme. I am looking forward to working with different teams across ERYC as part of the 'huddles' due to having a varied previous employment background externally both within mental health services and supporting children and families with complex additional needs.

I choose to be a change champion as I have a passion for developing my skills and helping others. Hello, I am a care coordinator within the Haltemprice Community Wellbeing Team. I wanted to be a change champion as I really enjoy working with other teams and colleagues. I enjoy supporting colleagues in reflective huddles because I always learn so much from others. As a change champion I can share this learning with colleagues across all adult services.
Hi I am a Care Coordinator in the Beverley Community Wellbeing Team. As I was assigned to be the care coordinator at Rita Hunt Court I was able to take part in the initial pilots for the maximising independence workstream prior to COVID. I wanted to be a change champion to share the learning from the pilot and to support colleagues adapt to changes post COVID while still maintaining the strength based practice we are all so good at.
The programme team, in collaboration with IMPOWER, have been rolling out ADKAR Change management Training to senior leaders and managers across adult services and business management and commissioning. The ADKAR change management framework, which stands for Awareness, Desire, Knowledge, Ability and Reinforcement, looks to support staff leading on change in East Riding to understand the process that they, and their teams, are going on, and give them the skills to embed change effectively.
Over the last year Connect to Support has been transformed to a more modern offer for staff and the public. It now boasts an up to date and accessible Community Directory, a PDF booklet of your favourites that can be emailed to the people you are supporting and their carers, a professional’s zone that will support staff and, not to mention a whole host of advice, information and guidance on how to support people to do more for themselves and maintain and maximise their independence. Connect to Support will also help promote a clearer relationship between the public and what adult social care actually is. The contribution from champions in the development of Connect to Support has been invaluable. In order for Connect to Support to become the first point of contact for the workforce and public around all things adult social care, we want you to be involved in the next stage of its development. From mid-October through to January 2021 we will be asking the workforce to use Connect to Support and feedback what works well, what is missing, what do you need it to do, what would you like it to do, does it look and feel right? Connect to Support is part of your tool kit and it is important that it does what you need it to. Your team managers and champions will soon receive an invitation to a presentation of the new Connect to Support, where we will share details on how feedback can be given, and how your suggestions and ideas will be developed on Connect to Support going forwards.

Over the last month we have been attending team meetings to provide you with further information on the change programme and to give you an opportunity to ask questions. Thank you all for the positive response that has been shown so far. Teams have put forward some good ideas that we will consider or incorporate into our planning for the programme. We are aware that we haven't managed to get out to all teams yet, but we are in the process of setting up meetings in October and November.
