NEW SEND Transition Guides
 As we approach the end of the academic year, it is essential to implement effective transition support for those children who will be moving to primary school and for our children and young people moving on to secondary and post 16 education.
The Cheshire East Transition Team has been working collaboratively with all stakeholders to support school transitions and promote a consistent approach across educational settings. In response to feedback and a recognised need for clear, structured resources, they have developed three comprehensive transition guides - each tailored to a key phase of transition.
These guides are now available on the Cheshire East Local Offer website, alongside dedicated support pages designed to assist families throughout the transition process.
We encourage all professionals to visit the Cheshire East Transitions Webpage and explore the Graduated Approach to Transition - a structured framework to guide your planning and support strategies.
Key Actions for This Term:
- Continue to identify children who may benefit from an Enhanced or Personalised Transition.

Assess individual needs to ensure the appropriate level of support is in place.

Special Educational Needs and Disabilities (SEND)
Neurodevelopmental Pathways – new proactive communication resources for Cheshire East
The Cheshire and Merseyside NHS Integrated Care Board in Cheshire East Place is pleased to announce new proactive support resources for neurodivergent children, young people, and their families, regardless of where they are on their neurodevelopmental journey.
We commissioned the national charity Contact, in partnership with co-production specialists Genuine Partnerships and the Cheshire East Parent Carer Forum, to co-produce a suite of new materials. These resources were developed together with parents, practitioners, and neurodivergent young people.
The project was led by a neurodivergent graphic designer and a Cheshire East-based project manager, both of whom are parent carers with lived experience.
The outcomes include:
- A new web platform featuring a searchable directory of local and national resources.
- Information on personal passports, current referral pathways, and future service developments.
- Redesigned practitioner guidance and operational communications, co-produced with families.
Every child and young person has unique needs and the support provided to them should be tailored to respond to this. This initiative aims to empower families with clear, accessible information so they can make informed decisions and access the right support, at the right time.
Summer activities/support across Cheshire East
Support for children and young people continues across the locality throughout the summer holidays. Cheshire East in collaboration with NHS and CVS have a range of events taking place including:
- Little Stars Alsager (SEND)
- Ready, Steady, School (North)
- JIGSAW North/South (age 11-25)
- Children's Activity Day (Oakenclough Family Hub)
- SEND Health Practitioner Drop-in
- Talking Walk-In (Speech and Language drop-in)
- Axess 4U - Free walk-in sexual health service for age 19 and under

Children and Young People's Activity Day
To celebrate International Youth Day and the support you can find in our Family Hubs, the Children and Young People’s Trust Board and Early Help Services are hosting an activity day for children, young people (0-19 or 25 if SEND or Care Leaver) and their families.
Come along to have fun, try something new and tell professionals what they can do to improve the services and support that you receive.
Venue: Oakenclough Family Hub, Colshaw Drive, Wilmslow, SK9 2PZ
Date: 14th August 2025
Time: 11pm-3pm
A light lunch and refreshments will be provided.
Activities for all ages include:
Sports, arts and crafts, circus skills, mocktails and much more

Healthy Start
 Help is available in the north locality to apply for the Healthy Start card at a drop in at Bromley Farm Wellbeing Hub.

The Bread and Butter Thing comes to Alsager!
The Bread and Butter Thing will be starting in Alsager on Monday 30th June at the Cricket Club
The Bread and Butter Thing will be starting in Alsager on Monday 30th June at the Cricket Club and will be open from 12pm (Fairview Avenue, Alsager, Stoke-on-Trent ST7 2NW).
They already have quite a few volunteers but would love to have some more to help out each week, and they need people to help pack the bags and distribute them to those coming along. They need people to be there from 10 – 1pm each Monday.
