Accident Prevention Reports Due April 1

Insurance companies with Texas workers’ compensation insurance policies effective in 2016 must file their DWC Form-109, Accident Prevention Services Annual Reports by April 1. The DWC Form-109 must be submitted by email to aps@tdi.texas.gov. Contact DWC at 512-804-5020 for questions.
