New Maternal RSV Vaccine Available in the TVFC Program

Ordering of Pfizer’s bivalent RSVpreF vaccine (Abrysvo™) in the Vaccine Allocation and Ordering System (VAOS), begins Wednesday, December 20, 2023.

Ordering

Providers should only order vaccines they anticipate they can administer before the Advisory Committee on Immunization Practices (ACIP) recommendation of January 31, 2024. Be mindful of the amount ordered because any unused doses must be stored until the following RSV season.

Vaccines administered past January 31, 2024, must be reported to VAERS as an administration error.

Recommendations for Use of Abrysvo Vaccine

Abrysvo is recommended for active immunization of pregnant women during weeks 32 through 36 of pregnancy using seasonal administration (September 1 through January 31) to prevent lower respiratory tract infection (LRTI) caused by RSV in infants from birth through six months. One 0.5 mL dose should be administered intramuscularly (preferably deltoid region of the upper arm).

Maternal RSV Vaccine TVFC Program Eligibility

Pregnant women 18 years and younger who meet one of the following eligibility criteria may receive TVFC vaccine:

  • Enrolled in Medicaid or Medicaid-eligible
  • Uninsured
  • American Indian or Alaskan Native
  • Enrolled in the Children’s Health Insurance Program (CHIP)

Underinsured children are only eligible for TVFC vaccines to the extent their insurance does not provide coverage and is defined as a child who has health insurance:

  • But the coverage does not include vaccines
  • That covers only selected vaccines
  • That sets a fixed dollar limit or cap on vaccine coverage.

The CDC only covers costs and provides doses for immunizing underinsured VFC clients at DSHS Public Health Regions, Local Health Departments, Federally Qualified Health Centers, and Rural Health Clinics. Underinsured patients must be referred to one of the following, federally reimbursed, locations to receive the RSV vaccine, Abrysvo™, under the TVFC program:

  • DSHS Public Health Regions (PHR)
  • Local Health Departments (LHD)
  • Federally Qualified Health Centers (FQHC)
  • Rural Health Clinics (RHC)

Provider Vaccine Choice

The Texas Department of State Health Services (DSHS) Immunization program has updated providers’ vaccine choice in VAOS for Abrysvo™. Providers can adjust their vaccine choice as needed. To update vaccine choice, follow these steps after completing the required reporting:

1. On the order screen, select the facility from the drop-down menu, verify the required conditions, if applicable, list out-of-office dates click “Next” at the bottom of the screen.

2. Click “Update Choice”. Check the applicable vaccine. Click “Submit”.

3. Return to the order screen. Enter requested vaccine quantity (list a “Reason for Deviation” if the quantity requested is greater than your suggested quantity). Click “Review Order,” then “Submit,” to complete the vaccine order. 

ImmTrac2 Reporting

The TVFC program policy requires provider report to the Texas Immunization Registry (ImmTrac2). Texas Health and Safety Code §§161.007-161.009 requires all medical providers to report all immunizations administered to clients who are 17 years of age and younger to ImmTrac2 within 30 days of administration.

TVFC Program Administration Fee 

 

TVFC program providers can charge an administrative fee of up to $13.75 per dose for Abrysvo™ for non-Medicaid/non-CHIP patients. TVFC program providers must not: 

  • Charge an administration fee to Medicaid or CHIP patients for receiving vaccines
  • Deny administration of a TVFC vaccine to an eligible child because of the child’s parent or guardian’s inability to pay 
  • Send a patient to collections or charge additional fees for non-payment of a TVFC administration fee 

For Medicaid children, the clinic site must accept the reimbursement for immunization administration fee set by the state Medicaid agency or the contracted Medicaid health plans.

 

Patient Eligibility Screening 

 

The TVFC program requires providers screen all patients at every immunization encounter. Providers must document eligibility screening at every visit to ensure that TVFC program vaccine is used only for TVFC-eligible population. The TVFC program requires full compliance on screening for eligibility.  

 

Eligibility screening must include the following:

  • Date of screening  
  • Child’s name 
  • Child’s date of birth 
  • Parent/guardian’s name 
  • Clinic name 
  • Eligibility status for each visit 

Providers must keep eligibility screening records on file with the patient’s record, for a minimum of five years after the last date of service to the patient and must be easily retrievable. 

 

Monthly Reporting 

 

Providers must submit the following reports in VAOS by the seventh of each month to order TVFC vaccines.  

 

Required: 

  • Temperature logs
  • Doses administered
  • Physical inventory

 

If applicable: 

  • Vaccine loss
  • Receipt of vaccine shipments
  • Vaccine transfers

 

Providers must submit monthly reports in VAOS even if vaccines are not ordered during the reporting month. Once the required reports have been submitted, VAOS will automatically lift ordering holds.
