Reminder for NFs That Received COVID-19 in Healthcare Relief Grant “Tier 2”: Required Reporting Due on Aug. 31, 2023

Nursing Facilities (NFs) that were awarded funds for the Tier 2 COVID-19 in Healthcare Relief Grant for NFs (NF-CHRG, RFA #HHS0011337) must complete and submit their required Funding Utilization Report by Aug. 31, 2023, at 5:00 p.m. CDT.

Providers that received this award are encouraged to complete and submit this report before the deadline to avoid recoupment of a portion or all of their awarded funds. The report can be submitted any time between now and the deadline.

Reporting Portal

Awarded NFs can access the NF-CHRG reporting portal via SurveyMonkey.
