The Department of State Health Services (DSHS) has free laminated adult and children CDC vaccine schedules available online. 

To order a laminated schedule, create an account at www.immunizetexasorderform.com. Once an account is set up, you can search the catalog and add the vaccine schedules to your cart. 

Use the following search terms to find the laminated schedules in the Immunization Publication Order Form: 

  • 6-105L: "Recommended Immunization Schedule for Children and Adolescents Aged 18 years or younger, United States, 2023 - Laminated (English)." 
  • 6-104L: "Recommended Adult Immunization Schedule for Ages 19 or older, United States 2023 - Laminated (English)."
