Guidance to Pharmacists for COVID-19 Vaccinations Under 3 Years
Per Texas State Board of Pharmacy (TSBP) guidance, for patients 6 months - under 3 years of age, a pharmacist may administer the COVID-19 vaccine pursuant to a delegated act or a written protocol under state law. A pharmacist may administer the COVID-19 vaccine to a patient under 3 years of age only upon a referral (e.g., prescription) from a physician with an established physician-patient relationship.
Vaccine providers are encouraged to visit the Texas State Board of Pharmacy's webpage, for more information on vaccinating children younger than 3 years.
Monkeypox & COVID-19 Vaccination Schedule
Smallpox/monkeypox vaccine (JYNNEOS™) may be given at the same time as other vaccines. Certain people at increased risk of myocarditis (swelling of the heart muscle), including adolescent or young adult males, might consider waiting 4 weeks after JYNNEOS™ vaccination before getting an mRNA COVID-19 vaccine.

COVID-19 Pediatric Vaccine FAQ’s
Below are some frequently asked questions relating to the COVID-19 Pediatric Vaccine:
Q: Can a patient ages 5 years – 17 years who chooses Moderna as a primary series receive Pfizer orange cap (5 – 11) or Pfizer gray cap (12 y+) for a booster dose 5 months after completion of the Moderna primary series? A: Currently Moderna Primary ages 5 – 17 years are NOT eligible for an authorized booster.
Q: What do I do if a patient misses the second dose, and comes in for second vaccine weeks/months overdue? A: Doses administered at any time after the recommended interval are valid.
- Any COVID-19 vaccine dose administered at any interval after the recommended interval.
- Do not repeat the dose. There is no maximum interval.
- This deviation from CDC guidance does not require VAERS reporting.
Q: How soon can a 4 y/o get the Pfizer orange cap? A: As soon the 4-year-old turns 5. Vaccination providers should ensure the correct age-appropriate product is administered based on the recipient’s age on the day of vaccination. Vaccine doses should be administered by the intramuscular route and in accordance with the recommended intervals for that age group. Q: For Pfizer 6 months – 4years: What is the guidance if a child is 4 years going on 5 years within the time, they are getting the series? Do they receive 3 doses of maroon cap vial regardless of age? Do we give 3 doses, and base which cap color on their age? A: Children who started a primary series and turned from age 4 – age 5 years between dose 1 and dose 2 in the primary series should receive the dose appropriate for their age group. If the child is 4 years old when receiving the first dose, they should receive Pfizer maroon cap. If at dose 2 they are 5 years old, give them Pfizer orange cap.
Q: Are COVID-19 vaccines interchangeable for adults? A: Complete the primary series with same product. If the vaccine product previously administered cannot be determined or is no longer available, any age-appropriate mRNA COVID-19 vaccine product may be administered at least 28 days after the first dose. Any COVID-19 vaccine product (age appropriate) may be administered for a booster dose. It does not need to be the same product used for the primary series.
Reminder: Ancillary Kits Requirements for Pack-Size Orders
For all pediatric (6 months – 5 years of age) COVID-19 vaccine orders, pediatrics ancillary kits containing needles, syringes, diluent, alcohol swabs, limited PPE, and shot cards will be automatically added to a vaccine order and will ship at the same time as the vaccine. Providers are unable to opt out of receiving ancillary kits for any COVID-19 vaccine that require diluent.
Providers cannot opt out of ancillary kits for the following vaccines:

Providers can opt out of ancillary kits for the following vaccines:

Moderna COVID-19 vaccine (children 6 months – 5 years of age) supplied in a multiple dose vial with a dark blue cap and a label with a magenta border.
Reminder: COVID-19 Pediatric (6 months – 5 years) Vaccine Open Ordering!
The following COVID-19 pediatric vaccines are now available for open ordering through the Vaccine Allocation & Ordering System (VAOS):

Pfizer COVID-19 vaccine (children 6 months – 4 years of age) supplied in a multiple dose vial with a maroon cap and a label with a maroon border.

Moderna COVID-19 vaccine (children 6 months – 5 years of age) supplied in a multiple dose vial with a dark blue cap and a label with a magenta border.
A full pack-size order for Moderna Pediatric COVID-19 vaccine (6 months – 5 years) and Pfizer Pediatric COVID-19 vaccine (6 months – 4 years) is 100 doses, or 10 vials. Providers who want to place a smaller order should make those orders in one-vial, 10-dose increments.
Considerations prior to reporting:
- As per COVID-19 policy, providers who have not reported their monthly inventory will be unable to order vaccine until they have completed this reporting requirement in VAOS.
- Ensure your data logger certificates comply. Current data logger certificates are required to place orders in VAOS.
