COVID-19 UPDATE: MBCC Temporary Signature Flexibilities Extended

As part of its continued response to novel coronavirus disease (COVID-19), the Health and Human Services Commission (HHSC) has extended the period for temporary signature flexibilities for Medicaid for Breast and Cervical Cancer (MBCC) services. Previously, HHSC allowed the use of digital/electronic applicant and contractor signatures on the H1034 – Medicaid for Breast and Cervical Cancer application form from March 30, 2020, to April 30, 2022. The termination date for these flexibilities has now been extended through July 15, 2022.

As a reminder, Breast and Cervical Cancer Services (BCCS) contractors may assist women with applying for MBCC benefits over the phone or through other remote means to complete the H1034 application.

Applicant signature requirements:

  • The applicant may sign the H1034 application (page 5) using a digital/electronic signature.

  • If the applicant is able, she may print the H1034 application, add her handwritten signature, and either scan and email or send via fax to the contractor.

BCCS contractor signature requirements:

  • The BCCS contractor may sign the H1034 application (page 6) using a digital/electronic or handwritten signature.

If you have any questions about this temporary policy modification, or you need to request a PDF copy of the H1034 form, please send an email to BCCSProgram@hhs.texas.gov.
