Deadline Extended to COB Jan. 9 - ALF COVID-19 Vaccination Data

Assisted living facilities have until close of business on Jan. 9, 2021 to complete the COVID-19 Vaccination Survey. HHSC Long-term Care Regulation and the Texas Department of State Health Services extended this deadline.

LTCR and DSHS request that all assisted living facilities complete the survey. A provider that owns multiple licenses or locations would submit separate surveys for each building. This will inform LTCR and DSHS:

  • On COVID-19 vaccines reaching assisted living facility staff and residents.
  • Enrollment status in the state or federal COVID-19 vaccine partnership program.
