The Texas Early Hearing Detection and Intervention (TEHDI) Program within the Department of State Health Services (DSHS) requests your assistance. During the 82nd legislative session, House Bill 411 passed making changes to Chapter 47 of the Texas Health and Safety Code. Section 47.003(a)(1) requires the creation of a form to document a parent's decision to decline the required newborn hearing screening at birth. DSHS is soliciting stakeholder input on the attached draft form. Once stakeholder input is obtained, DSHS will post the form on the TEHDI Program’s website for birthing facilities to access and use as needed.
The TEHDI program is looking to you as a stakeholder in the hearing screening, diagnosis and intervention process to review the suggested content and layout of the form.
Written comments, edits or feedback regarding the form can be provided via email to: sarah.shaw@dshs.state.tx.us. Comments or feedback will be accepted through December 28, 2011.
Thank you for your help.
