Changes to Disabled Veteran License Plate, Form VTR-615

We are informing you of a recent design change to the Application for Disabled Veteran License Plates and Parking Placards (Form VTR-615). The form content was expanded to meet requirements set by the Texas 85th Legislature. Changes to the form include adding 23 new Disabled Veteran license plate options.

The Application for Disabled Veteran License Plates and Parking Placards (Form VTR-615) is now a three-page application inclusive of the instructions and eligibility requirements. The form will be available on the Texas Department of Motor Vehicles (TxDMV) website on September 1, 2017.

No changes were made to the Disabled Veteran license plate fees. Counties are required to submit the applications and supporting documents to the TxDMV Special Plates Unit for archiving.
