 PCPCH Standard 6.B encourages primary care practices to connect their patients with opportunities for self-management of their conditions and overall health.
For patients with arthritis, Walk With Ease offers an online, self-directed six-week walking program that is free to all Oregonians and does not require an official referral! Walk With Ease is proven to reduce the pain and discomfort of arthritis, increase balance, strength and walking pace, build confidence in the ability to be physically active, and improve overall health. In addition, regular walking is associated with lower risk of cardiovascular disease and improvements in mood, cognition, memory, and sleep.
Physical education and arthritis ready-made educational materials and posters that you can use to promote physical activity and Walk With Ease are available on the Oregon Medical Association’s Physical Activity and Arthritis Toolkit page.

OHA's Transformation Center in collaboration with Boost Oregon are hosting in-person motivational interview trainings with no-cost continuing medical education credits. This comprehensive workshop is designed to equip health care providers with essential skills for effective vaccine discussions. It offers an in-depth introduction to the principles of Motivational Interviewing (MI), focusing on its application to conversations about vaccines.
Participants will engage in interactive activities and real-world scenarios that illustrate the core concepts of MI, emphasizing collaboration and empathy. They will learn to skillfully use questions and reflective listening to encourage meaningful dialogue with patients. Additionally, the workshop covers approaches for providing information and advice that align with MI principles, ensuring that interactions are supportive and patient centered. Continuing medical education: The American Academy of Family Physicians (AAFP) has reviewed Navigating Vaccine Conversations: The Motivational Interviewing Approach and deemed it acceptable for AAFP credit. Term of approval is from 10/01/2025 to 10/01/2026. AAFP Members should claim only the credit commensurate with the extent of their participation in the activity.
Correction made to summary of PCPCH TA Guide Version 3
The Patient-Centered Primary Care Home (PCPCH) Program has issued a correction to the updated version of the PCPCH 2025 Technical Assistance Guide which was released last month. Please review page 2 of this summary document regarding Measure 3.C.3 (Integrated Behavioral Health Care Providers).

Patient-Centered Primary Care Homes (PCPCHs) are health care practices that have been recognized by the Oregon Health Authority (OHA) for their commitment to providing high quality, patient-centered care. The PCPCH Program administers the application, recognition, and verification process for practices applying to become Patient-Centered Primary Care Homes.
