New report available on impact of the PCPCH program!
The Oregon Health Authority (OHA) has released the 2024-2025 Patient-Centered Primary Care Home Program Impact Report. This report provides an overview of the program's activities and impact on the health system over the past two years. Highlights include:
- Updates to the PCPCH program standards that align with OHA’s strategic plan and help clinics build capacity to identify and address health inequities in their patient population
- How the PCPCH program standards influence the way providers and staff care for patients and what services clinics offer
- Results from an evaluation showing that the program has significantly reduced costs in the health system and positively impacted utilization
- Program-led webinars, learning collaboratives and education resources for clinics and community partners
- Clinic experiences with site visits and practice coaching from PCPCH program staff – read the clinic quotes!

About the Patient-Centered Primary Care Home Program
Patient-Centered Primary Care Homes (PCPCHs) are health care practices that have been recognized by the Oregon Health Authority (OHA) for their commitment to providing high quality, patient-centered care.The PCPCH Program administers the application, recognition, and verification process for practices applying to become Patient-Centered Primary Care Homes.The program is also working with partners across Oregon to support adoption of the primary care home model. The mission of the PCPCH Program is to be a trusted partner in primary care, collaborating with stakeholders to set the standard for transformative, whole-person, and evidence-based care.
