More practices awarded the Health Equity Designation!
 We're excited to announce that 11 more primary care practices have been awarded the PCPCH Health Equity Designation, including 8 pediatric practices and 3 Federally Qualified Health Centers (FQHCs)! This designation is awarded to practices that demonstrate a special commitment to providing high-quality of care to patients from diverse backgrounds and circumstances. PCPCHs can earn this designation by meeting at least 15 out of the 20 equity-focused measures in the 2025 PCPCH model. We look forward to learning about how other practices throughout the state are engaging in this work!

The Oregon Health Authority's Transformation Center is hosting a free webinar for Coordinated Care Organization (CCO) staff and providers working on the CCO incentive metric: Assessments for Children in ODHS Custody. This is one of the many quality measures that PCPCHs can choose to meet the required PCPCH Standard 2.A - Performance & Clinical Quality, and practices may also find the webinar useful for some of the optional PCPCH measures such as 5.E.3 (care coordination with government systems). The webinar will offer an introductory overview of the purpose and value of each required assessment, provide scripts for communicating effectively with resource families, and highlight current systems and practices in place across Oregon. The session will also spotlight a partnership between a CCO and clinic that is successful in consistently coordinating on-time assessments. Click here to register. PCPCH Standard 6.B - Education & Self-Management Support recognizes practices that empower their patients to manage their own health and wellness with support from their families and caregivers through patient education and self-management support. Below are a couple self-management resources that may be useful to patients hoping to improve chronic conditions or quit smoking.
Free walking program for patients in Oregon
Walking is a wonderful activity patients can do alone or with family and friends and is known to boost mood and mental health, reduce pain, and supports cardiovascular health and a strong immune system. For patients who can walk safely, the Walk With Ease self-directed, online six-week walking program is a great addition to any treatment plan. The program is free to all people who live in Oregon and does not require a referral. Patients can start today by clicking “register online” at this link. Free "Freedom from Smoking" workshop for patients in Oregon
 (First session Monday, September 29 at 5:30 pm)
Although the first steps can be hard, finding your path to a smokefree life is easier with the right help. The American Lung Association’s Freedom From Smoking program is a proven way to quit smoking, and stay quit, even if you’ve tried before and went back to smoking. The Samaritan Health Services Health Education Department via the Regional Health Education Hub is offering this hybrid online and in-person workshop. Participants will learn how to decrease cravings, create their new self-image, and develop an action plan for their "quit day.” The workshop involves 8 sessions (1.5 hours each), with the first on Monday, September 29 at 5:30 pm. Join colleagues from across Oregon working in HIV prevention and care for a free, interactive online conference focused on thoughtful collaboration and practical solutions for ending new HIV infections. This interdisciplinary event will offer dedicated tracks for clinicians, case managers/social workers, administrators, and public health professionals. Together, participants will explore strategies to strengthen systems of care that support whole person health. Click here to register.
