Dear School and District Leaders,

The message below is the first of several messages that will be coming from the Oregon Health Authority regarding public health requests. Please forward this important opportunity to your K-12 teachers. It is their choice to participate or not, but every teacher should have access to this invitation.

Oregon Health Authority (OHA) continues to conduct statewide COVID-19 surveillance. As part of this surveillance, OHA is inviting you to participate in a K-12 teacher COVID-19 surveillance project. Your blood will be tested for COVID-19 antibodies now (spring 2021), then again in the summer of 2021. If your blood test is positive for COVID-19 antibodies, the laboratory will be able to determine if the antibodies are from COVID-19 vaccination, or from SARS-CoV-2 infection. 

If you register as a participant, you will receive a blood collection kit in the US mail (a fingerstick and collection cards) with complete instructions and a postage-paid return envelope. You will also receive your results via US mail. It may take up to one month for your results to be available.

Participation is completely voluntary, and your test results will not be shared with your employer or the Department of Education. 

If you would like to participate, please complete the appropriate electronic registration form. There are two forms to choose from:

  • Complete this form if you provided any Hybrid or In-Person Learning through March 26, 2021.
  • Complete this form if you have provided ONLY Comprehensive Distance Learning (CDL) through March 26, 2021.
