Meeting of Northwest Health Foundation (NWHF) Patient-Centered Primary-Care (PCPC) Task Force

In partnership with the Oregon Health Authority, Northwest Health Foundation has convened a diverse group of clinicians (primary care, mental health), patients, public health, and healthcare delivery technical experts to provide recommendations that will inform an approach to support broad implementation of Patient-Centered Primary Care Homes (PCPCH) in our state. The taskforce will develop recommendations for infrastructure and technical assistance and prioritize action steps for broad-scale implementation of the PCPCH in Oregon.

The first meeting of the NWHF Patient-Centered Primary-Care Task Force will be held on Thursday, July 14, 2011, 9:30 am - 2 pm at NWHF, 221 NW 2nd Avenue, Suite 300, Portland, Oregon.
