Coordinated Care Organizations (CCOs) provide care for Oregonians enrolled in the Oregon Health Plan/Medicaid. CCOs are the foundation of Oregon’s vision to improve care, achieve better health and lower costs. CCOs are networks of all types of health care providers (physical health care, addictions and mental health care and sometimes dental care providers) who have agreed to work together in their communities to serve people who receive health care coverage under OHP. CCOs are focused on prevention and helping people manage chronic conditions like diabetes and asthma. This helps reduce unnecessary emergency room visits and gives people support to be healthy. Innovator Agent positions (OHA12-0344) - Open until further notice

Seven positions will provide the critical leadership necessary as they become the single point of contact between each CCO and the Oregon Health Authority.  These positions will champion and share health care system innovation ideas with the CCO and OHA as Oregon works to transform health and health care in the state.

The Transformation Center Director position (OHA12-0345) - Open until filled

The person in this position will provide the critical leadership necessary to ensure a high-performing health system for Oregon by supporting internal and external health care transformation efforts, including CCOs, who assist OHA to achieve its vision of a Healthy Oregon.
