Beginning in January of 2025, all practices applying or re-applying for PCPCH recognition will need to do so under the 2025 PCPCH Recognition Criteria. Program staff are hosting a webinar series to help practices understand some of the revisions and additions under the new model. This is a great opportunity to familiarize yourself with the new model and ask questions about specific standards! Health Equity Designation (December 6, noon-1pm)
The Oregon Health Authority has set a strategic goal of eliminating health inequities by the year 2030, and the PCPCH program is uniquely positioned to implement strategies that support the achievement of this goal. In this session, PCPCH Program staff will provide an overview of a brand new special designation under the PCPCH model – the Health Equity Designation – including the criteria for recognition, the recognition process, and other important information.
Office Hours with PCPCH Staff (January 17, noon-1 pm)
As practices prepare to apply under the new 2025 version of the PCPCH model and standards, PCPCH staff will be hosting office hours for anyone that would like additional guidance on the standards, criteria, application process, or any other program-related topics. We encourage all of our practices to use our team as a resource!
Standard 2.B – Value Based Payment (January 24, noon-1pm)
Value-based payment (VBP) is one of OHA’s primary strategies for achieving the triple aim of better health, better care and lower costs for all Oregonians. VBP can support PCPCH’s team-based care and care management efforts that otherwise can’t be reimbursed via a traditional fee-for-service payment system, such as outreach, traditional health workers, partnerships with community entities, and other non-billable types of care. In this session, PCPCH Program staff will review the brand new Standard 2.B under the 2025 model which focuses on establishing various types of VBP arrangements with payers based on the Health Care Payment and Learning Action Network (HCP-LAN) framework.

Patient-Centered Primary Care Homes (PCPCHs) are health care practices that have been recognized by the Oregon Health Authority (OHA) for their commitment to providing high quality, patient-centered care.The PCPCH Program administers the application, recognition, and verification process for practices applying to become Patient-Centered Primary Care Homes.The program is also working with partners across Oregon to support adoption of the primary care home model.
