Beginning in January of 2025, all practices applying or re-applying for PCPCH recognition will need to do so under the 2025 PCPCH Recognition Criteria. Program staff are hosting a webinar series to help practices understand some of the revisions and additions under the new model. Standard 1.E – Electronic Access (April 26, noon-1pm)
Patients' various backgrounds and needs can impact their familiarity with, and ability to access, their health information through electronic mediums. In this session, PCPCH Program staff will review some of the overall revisions to Standard 1.E under the 2025 model such as new expectations around providing electronic information in an accessible format, the inclusion of a measure focused on patient portals (previously 1.G.1), and changes in measure points values. Standard 1.G – Alternative Access (May 10, noon-1pm)
As a result of system-wide changes implemented during COVID-19, telehealth has become a common alternative visit type that practices throughout the state offer. In this session, PCPCH Program staff will review some of the overall revisions to Standard 1.G under the 2025 model such as the parsing out of telehealth services into a distinct measure that includes considerations for patients with primary languages other than English. We will also review the addition of a new 15-point measure inspired by feedback from community listening sessions around providing care in community settings. Click here to register.
Standard 2.A – Performance and Clinical Quality (May 24, noon-1pm)
Many PCPCHs partner with CCOs and other agencies on quality metric goals and specifications, which can change over time. In this session, PCPCH Program staff will review some of the overall revisions to Standard 2.A under the 2025 model such as a more flexible menu of quality metrics for practices to choose from, intended to make it easier to align with other partners. We'll also review the addition of two new measures related to tracking and reducing disparities in quality measure performance, as part of OHA's commitment to supporting a primary care delivery system that meets the needs of those experiencing health inequities and barriers to utilizing primary care services. Beginning in January 2025, PCPCHs that partner with Traditional Health Workers (THWs) will be able to attest to meeting the new PCPCH Standard 6.E.3. The OHA Transformation Center recently released a Traditional Health Worker Payment Model Guide for organizations interested in integrating or advancing the use of THWs into their health and social care systems. The guide includes findings from interviews with Coordinated Care Organizations (CCOs) about existing integration of THWs in their systems, case studies of THW payment models from CCOs in regions across the state, and a review of various THW payment models.
