The Oregon Health Authority (OHA) Patient-Centered Primary Care Home (PCPCH) Program is recruiting for a Patient-Centered Primary Care Home Verification Specialist to support Oregon’s health system transformation goals by fostering improvement and innovation within the primary care delivery system. The PCPCH Verification Specialist provides consultation, technical assistance and evaluation of services for Oregon's PCPCH program to verify that primary care practices recognized as PCPCHs by OHA are meeting program requirements as specified by statute, administrative rules and interpretive guidelines.
As a PCPCH Verification Specialist, you will:
- Conduct site visits to clinics in compliance with PCPCH Program protocols to verify whether clinics are meeting the PCPCH program requirements.
- Examine clinic documentation for accuracy, completeness, form and timely submittal as part of the site-visit data collection and verification process.
- Conduct interviews with providers, administrators and other staff at the clinic as part of the site visit data collection and verification process.
- Analyze and evaluate data, documentation and other information collected during the site visit to determine if clinic is compliant with PCPCH Program requirements or if an improvement plan is needed.
- Collaborate with PCPCH Program team to write a post-site site visit report for the clinic which includes site visit findings (areas of compliance and non-compliance), an improvement plan timeline if needed, and deadlines for full compliance.
This position requires travel around the state, including occasional overnight stays. Work can be conducted remotely with occasional in office meetings in Portland, Oregon.
This position is a full-time, permanent, classified position which is represented by a union. It is open, competitive and will close on 3/26/24.
