Oregon Department of Human Services sent this bulletin at 03/16/2012 01:43 PM PDTThe State of Oregon, Oregon Health Authority, Office of Oregon Health Policy & Research, is seeking proposals to provide technical assistance to health care practices to facilitate the implementation of Patient Centered Primary Care Homes in Oregon.
