Review Your Immunization Numbers

To: School and Children’s Facility Administrators

The Oregon Immunization Program has collected the immunization numbers for the 2021-2022 school year. Before the information is available on the Oregon Department of Education school report cards and the Oregon Health Authority website, we ask that you check and confirm your numbers match our numbers.

How do I check my numbers?

Visit https://www.surveymonkey.com/r/6VJQKH3 to by Friday May 27^th. Compare the numbers from the immunization reports Sections E-H, that you submitted to the county.

NOTE: Do NOT use Google Chrome, you will not be able to review your numbers.

What do I do if the numbers don’t match our records?

Email oregon.imm@state.or.us with a copy of your report submitted to the county with the correct numbers circled.

Why should I check my numbers?

Immunization rates are publicly available, for all schools and children's facilities with 10 or more children. We want your information to be accurately shown.

What if I can’t find my site’s numbers in the survey?

• For confidentiality, sites with less than 10 children in the adjusted enrollment from the Sections E-H reports will not be displayed.
• The spreadsheet is organized alphabetically by county, then by site name.
