Healthy Heart Ambassador Program facilitator training now available
The Healthy Heart Ambassador Blood Pressure Self-Monitoring Program is a 4-month, evidence-based program led by Healthy Heart Ambassadors. These trained facilitators help participants to lower and manage their blood pressure by:
- Empowering participants to measure and monitor their blood pressure using proper technique,
- Providing individualized support and nutrition education for participants to learn about ways to eat healthier, and
- Guiding and encouraging participants to find their own path to better blood pressure management. They do not diagnose or treat high blood pressure.
The program can be held at a variety of locations; each site should have space for office hours and for the nutrition education seminars. The program can also be virtual.
- Y-USA and the Centers for Disease Control and Prevention (CDC) developed this program for use in non-YMCA community-based settings.
- The Living Well Program, part of the Public Health Division's Health Promotion and Chronic Disease Prevention section, is available to train Healthy Heart Ambassadors in Oregon.
