Fax or e-mail the completed form (Word or PDF) to us (fax 503-945-9908) as soon as possible.
Please make sure you are using the June 2011 version; you cannot complete 5010 registration using older versions of this form.
Third-party testing cannot proceed until we review and approve your new Exhibit C.

Updates to all 5010 Technical Specifications are now available

In all 5010 Technical Specification documents, the Interchange Receiver ID (ISA08) and Application Receivers Code (GS03) have been changed to ORDHS.
