 OLCC Commissioners approve sanctions and issue liquor license to Coffeehouse-Five

PORTLAND, Ore. – At their monthly meeting on September 16, the Commissioners of the Oregon Liquor Control Commission approved the following liquor license application:

            Coffeehouse-Five, 740 N. Killingsworth, Portland; Licensee is Coffeehouse-Five, LLC; David Wesley Nelson III, member.

The Commissioners ratified the following stipulated settlements:

Old Town Pizza Company, 118 NW Greenwood, Bend; will pay a fine of $2,970 or serve an 18-day liquor license suspension for two violations. The first violation is for permitting an employee to mix, sell, or serve alcoholic beverages, or supervise those who do, without a valid service permit. The second violation was for failing to verify the age of a minor before allowing her to buy or be served an alcoholic beverage when she reasonably appeared to be under 26 years old. Licensee is Old Town Pizza Co.; Terry Parker and Chrissie Parker, corporate principals.

Bottoms Up Bar & Grill, 3082 Lancaster Dr. NE, Salem; will pay a fine of $2640 or serve a 16-day liquor license suspension for two violations. The first violation was for permitting a minor to buy, be served or drink alcoholic beverages on the licensed premises. The second violation was for permitting a minor to be on the licensed premises or an area that is prohibited to minors. Licensees are Yaoya Chen and Dung Ngo.

Rising Sun Palace & Sunset Lounge, 2425 Oak Street, Baker City; will pay a fine of $1,815 or serve an 11-day liquor licenses suspension for failing to evict patrons when they knew the patrons had engaged in noisy, disorderly, or unlawful activities. Licensee is Rising Sun Palace, Inc.; Lik Fei Wong, corporate principal.

For an employee(s) failing to verify the age of a minor before allowing him/her to buy or be served an alcoholic beverage when he/she reasonably appeared to be under 26 years old:

Denny’s Restaurant, 1369 SE 1st Ave., Canby; will pay a fine of $1,485 or serve a nine-day liquor license suspension. Licensee will be removed from the Responsible Vendor Program. Licensee is Canby-Denn, Inc.; Scott Hamilton, Richard Squibb, Paul Fox, David Fox, Alicia Squibb, Cindy Fox, Khamla Fox, and Sarah Hamilton, corporate principals.

Bridge Street Market, 732 SW Bridge Street, Grants Pass; will pay a fine of $1,485 or serve a nine-day liquor license suspension. Licensee will be removed from the Responsible Vendor Program. Licensee is AMJO, LLC; Merrily Jo Nealy, member.

Dari Mart Store #13, 440 SW Western Boulevard, Corvallis; has paid a fine of $1,485. Licensee will be removed from the Responsible Vendor Program. Licensee is Dari-Mart Stores, Inc.; Gladys Gibson, Kathy Gibson, Kurt Straube, Karla Straube, Gary Straube, Joseph Gibson, Kimberly Gibson, Scott Gibson, Stephanie Gibson, Michael Gibson, Tani Gibson, and Warren Gibson, corporate principals.

The Hangout Sports Bar & Grill, 576 NE Burnside, Gresham; will pay a fine of $330 or serve a two-day liquor license suspension. Licensee will install age verification equipment and will use the equipment to verify the age of every person purchasing alcohol who reasonably appears to be under 26 years old. Licensee is One Twenty Eight, Inc.; Montgomery Collins and Margaret Collins, corporate principals.

Hillsboro Mini Mart, 517 SW 1st Ave., Hillsboro; will pay a fine of $330 or serve a two-day liquor license suspension. Licensee will install age verification equipment and will use the equipment to verify the age of every person purchasing alcohol who reasonably appears to be under 26 years old. Licensees are Donald Crane and Hyong Crane.

Circle of Wolves/Wolf Creek Wine Sellers, 300 Front Street, Wolf Creek; will pay a fine of $1,485 or serve a nine-day liquor license suspension. Licensee is Circle of Waters, LLC; Lloyd Johnston, Nancy Johnston, Daniel Schilberg, and Karen Schilberg, members.

Shipyard Bar & Grill, 894 Commercial, Astoria; has paid a fine of $330. Licensee will install age verification equipment and will use the equipment to verify the age of every person purchasing alcohol who reasonably appears to be under 26 years old. Licensee is Merry Time Museum & Taverns, Inc.; Fred VanHorn and Christell VanHorn, corporate principals.

Erickson’s Thriftway, 315 W. 3rd Street, Prineville; will pay a fine of $1,485 or serve a nine-day liquor license suspension. Licensee is Ochoco Foods, Inc.; Ronald Bryant and Robert Eberhard, corporate principals.

Gee’s Family Restaurant, 987 W. Second, Prineville; will pay a fine of $3,795 or serve a 23-day liquor license suspension. Licensee is Zhu Brothers, Inc.; Ying Poy Zhu and Gen Bung Chu, corporate principals.

Big Island Kona Mix Plate, 680 SW Powerhouse Dr. #1004, Bend; will pay a fine of $1,485 or serve a nine-day liquor license suspension. Licensee is Big Island Kona Mix Plate, Inc.; Ricky Kim, Sheri Kim, and Shinobu Kuga, corporate principals.

Walgreens #10893, 1080 SW 1st Avenue, Canby; has paid a fine of $3,795. Licensee is Walgreen Co.; Gregory Wasson, Mark Wagner, Jason Dubinsky, and Margarita Kellen, corporate principals.

 

For permitting an employee(s) to mix, sell, or serve alcohol or supervise those who do without a service permit:

Trillium Café, 207 Oak Avenue, Hood River; will pay a fine of $1,155 or serve a seven-day liquor license suspension. Licensee is Trillium Café, LLC; Joseph Kirkwood, Clare Kirkwood, Benjamin Vandehey, Danny Vandehey, and Roberta Vandehey, members.

Riley’s Pub, 5836 SE 92nd Avenue, Portland; has paid a fine of $1,485. Licensees are M. Kathleen Barnes and Philip Barnes.
