CDC FAQ COVID-19 Vaccinations First dose at Third Clinic

Pharmacy partners are hosting three on-site clinics at all long-term care facilities (LTCFs) enrolled in the federal Pharmacy Partnership for Long-Term Care (LTC) Program. CDC provided updated guidance in the Frequently Asked Questions (FAQ) on February 5, 2021 to help LTCF administrators and clinical leadership prepare for residents and staff who may get their first COVID-19 vaccine dose at the third and final clinic.

Please be advised that Walgreens has agreed to administer 1st dose COVID-19 vaccines at the 3rd and final clinic. OSDH will assist with the second doses for employees and residents who receive the second dose.


Provider Call Recording

The February 3rd, NF/SNF and the Non-Nursing Home/Licensed/ICF-IID Provider Call recordings are now posted and you may locate it on the LTC COVID Resource webpage.  Click on Provider Calls to access the recordings.

The webpage is still under construction and our Communications Department is working to get the revisions completed. If you need access to a call not listed, please email the LTC Survey Department at: LTC@health.ok.gov and indicate the week you are interested in. 


Change in Reporting NF/SNF Demographic Data

Intended audience: Nursing/Skilled Nursing Facilities

SNF Quality Reporting Program (QRP) quality data on Care Compare are updated on a quarterly basis.

The Centers for Medicare & Medicaid Services (CMS) will be transitioning to a new data source for providers’ demographic data for all five Post-Acute Care (PAC) provider types.  These demographic data include such items as the provider name, provider-mailing address, provider physical address, State, ZIP Code, etc.  These provider demographic data are displayed on the Provider and Quality Measure reports generated from the Quality Improvement and Evaluation System (QIES) Certification and Survey Provider Enhanced Reports (CASPER) Reporting application for SNF/NF providers. Additionally these same demographic data are displayed on the public reporting websites.

Historically provider demographic data have been maintained in the Automated Survey Processing Environment or ASPEN software; however, CMS will be transitioning to use the demographic information from Provider Enrollment, Chain and Ownership System (PECOS).  While this transition is underway, a final date when all demographic data will be obtained from PECOS has not been identified.  During this transition, all SNF/NF providers will be responsible to ensure their latest demographic data are updated and available in both the ASPEN and PECOS systems. 

Review the Provider Demographic Updates Document  which outlines the steps each NF/SNF provider should follow to update your demographic information in both PECOS and ASPEN systems. (The following information is applicable to all facilities) 

Any changes in facility information, including Administrator, DON, email, etc. must be reported to the Oklahoma State Department of Health using the ODH Form 958 Notice of Change form. 

Additionally, per Federal CLIA requirements 493.39(b) facilities must notify the CLIA Department directly whenever there is a change in facility information. If you have a change in ownership, a reorganization, or demographic information changes, or if the facility administrator is listed as the CLIA Director, and there is a change in administrators, then notify the CLIA Department at: (405) 426-8470 or email medicalfacilities@health.ok.gov.
