Employer Application
The business owner or employer contact must now sign the employer application. Agents cannot be listed as the employer contact or sign the application. Please use the updated form available on the Insure Oklahoma website.
Also, agents can no longer be listed as the primary user on the employer application. The business owner or employer contact must fill this role.
Agent of Record Form
The agent of record form has been updated. It now authorizes information sharing with agents but does not grant permission to make account changes. To allow an agent to make changes, the business owner or employer contact must assign them as an agent contact through the employer portal, where they will receive a unique user ID.
All individuals listed as primary contact, agent contact or employee contact will have access to sensitive account data and the ability to make changes. Each must have a unique user ID. Sharing IDs is strongly discouraged.
Please note that some corrections must still be made by phone, including correcting incorrect Social Security numbers on the employer staff list and incorrectly assigned quality benefit plans.
Effective as of September 2025, Insure Oklahoma has implemented a business process change to HIPAA-compliance verification for ESI businesses. Insure Oklahoma now uses only the federal employer identification number (EIN) for verification.

Quality Benefit Plan (QBP) assignments must align with employee eligibility dates.

Verify Eligibility Start Date: Always confirm the employee's eligibility start date.

Assigning the QBP: Do not assign a QBP until the month before the employee’s eligibility date. Example: If an employee's eligibility begins on March 1, the QBP should be assigned in February.
