All employer change forms must be signed by an active employer contact or the business owner listed on the groups account.
If the employer contact or business owner has changed, Insure Oklahoma requires a letter on company letterhead signed by the new business owner. The following must be included in the letter:
Last name
First name
Phone number
Fax number
Email address
User ID: If you are submitting for multiple groups, each group must have a unique User ID
If you add an employee to your staff list through the employer portal and make a mistake keying in the Social Security number, please contact the Insure Oklahoma helpline at 888-365-3742. You will not be able to update it on your own. This will help avoid having duplicate names with different Social Security numbers.
Insurance rate sheets must be the original document from the insurance carrier. Created documents are not accepted. Any time your rates change, you should submit these changes to Insure Oklahoma.
In addition, any time your business has an insurance carrier change or rate effective date change, the employer application must be submitted. The contact person or business owner’s signature is required on the application.
If you are not on the employee’s account as an authorized representative, we cannot discuss ANY details about their account with you. The employee can add you as an authorized representative by logging into their account and clicking the "Go" button in the change box on the bottom right.
For employers: This applies to those listed as a contact person or business owner.
For agents: This applies to those listed as Agent of Record (AOR).
For employees: We can only speak to the member if the account has an authorized representative listed to act on their behalf.
