2019 Addendum & Miscellaneous Training Updates

 

Providers will have an opportunity to make changes to their Addendum and Miscellaneous Trainings for calendar year 2019 (January 1 – December 31) starting on Friday, November 30TH.  OOD has had to move the date back from today until Friday due to the issue with the Provider Management Program (PMP).  OOD is still working on finishing updates the current Addendums so the website can update with the current form/format beginning on Friday.  The renewal cycle will close at 12:00 PM on December 21, 2018.  Submissions received after that will be processed and posted to the website after January 1, 2019.  Submission of an Addendum does not guarantee that OOD will approve/accept a proposed service.

 

Renewal Instructions

  1. You may download your current Addendums & Miscellaneous forms from Provider SEARCH (after 11/30/18). If you do not have a current Addendum or Miscellaneous Training form you can download one from PMP when you add the service.
  2. Make changes to the current form. Changes may include:
    • Add/Remove Service Descriptions/Rates – If you need to remove a service you may delete the service table if you need to add service you can copy and paste a service table and then edit it to add a new service description and rates.
    • Update Trained Staff –You can remove staff who are no longer with your agency or add new trained staff member names. If you are adding a staff person you must include a copy of their certificate to crpvendor@ood.ohio.gov when you submit your updated Addendum.
    • Propose New Trainings – Training requirements can be found on the Miscellaneous Training form in PMP. For 2019, the topics have remained the same (SEE LIST BELOW):
      • Computer Skills (One training to teach basics of MS Word applications)
      • Customer Service
      • First Aid/CPR
      • Food Service
      • General Office/Medical Office Clerk
      • Home Health/Child Care
      • Janitorial/Maintenance
      • Retail Sales
      • Warehouse/Forklift
  1. If you are adding a new service you will also need to log into PMP and add the service and submit your application. Please do this as soon as possible.  OOD has 45 days to review and approve Provider profile/service changes.
  2. Once you have finished making adjustments you will need to email the unlocked Word document(s) (either the Addendum, Miscellaneous Training, or both) to the crpvendor@ood.ohio.gov with the subject line of “2019 Renewal.”
  3. If you are not making any changes you must still email crpvendor@ood.ohio.gov so that OOD knows you have had the opportunity to make updates.
  4. Services that are not renewed or if OOD is not notified that there are no changes will be removed from Provider profiles prior to January 1, 2019.

The services described are funded, in part, with Federal funds awarded by the U.S. Department of Education (DOE) under the Vocational Rehabilitation (VR), Supported Employment Services, and the Independent Living Services for Older Individuals Who are Blind (OIB) programs. For purposes of the VR program, the Federal VR grant paid 78.7% of the total costs of the program. In Federal Fiscal Year (FFY) 2018, the VR agency received $100,336,097 in Federal VR funds. Funds appropriated by the State paid 21.3% of the total costs $27,155,767 under the VR Program.
