As mentioned in the earlier email from Superintendent DeMaria and Director Himes, below is a list of local health department contact names and email addresses. We are providing this so that you can connect with your local health department for reporting of COVID-19 cases among students and staff. When reporting school cases, please use this reporting template.
If you are not sure which local health department you should reach out to, please consult this list, which matches schools by geography to a corresponding local health department. If you already have an established relationship with your local health department, please continue to work with your existing contacts for infectious disease reporting.
