Criminal Conviction Determination

To determine if a criminal conviction would disqualify an individual from obtaining a registration and/or firearm endorsement, to work as a security guard or private investigator, a criminal conviction determination form can be submitted prior to submitting an application to PISGS.

To request a determination of disqualification, visit PISGS website at www.pisgs.ohio.gov, and click on the link Criminal Conviction Determination.

In the second paragraph click on Submit A Form, or at the bottom of the page click on Criminal Conviction Determination Form. Download and complete the entire form and submit with the $25 fee to the address listed on the document. PISGS will contact the licensed company for further instruction once a determination is made.
