Reminder to Licensees:

How to Make a Registrant's Name Change

At times, licensed providers must document legal name changes for their registrants. This can be done by logging into their PISGS Online company page and following these three simple steps:

STEP 1

Log into your PISGS Online company page, and select the "Replacement Card" option on the left navigation bar.

Note: A new registration card is required to be issued when information
on the card has been changed.

STEP 2

Search for, then select the appropriate registrant record.

STEP 3

  1. Click the "Name Change" option.
  2. Make the appropriate change in the "New First Name" field.
  3. Make the appropriate change in the "New Last Name" field.
  4. Click on the "Browse" button to upload the supporting legal documents that verify the name change.
  5. Click on the "Add to Cart" button to complete the request and prepare for payment.

Note: The fee for a new registration card is only $5.
