Termination Requests
As per N.J.A.C. 7:28-64.4(f), a licensee who provides sufficient information for the Department to determine that a facility meets all release criteria shall be refunded half of the annual fee, if the information is provided to the Department during the first half of the fiscal year. The first half of the fiscal year ends on midnight of December 31. No refund is given if the information is provided to the Department after December 31.
Please get your termination requests in as soon as possible if you would like a refund of half of your annual fee. The termination form can be found here:
