Many factors can affect a client’s ability to schedule and get in for screenings. Your time is valuable and the EWM program provides compensation for addressing potential barriers to care.
EWM Program clients ages 35+ answer questions about any barriers that may impact preventive services, follow-up care and healthy behaviors. Clinics may have internal workflows that address these client barriers.
EWM Screening Card: Providers who address client barriers and document them on the EWM screening card during the EWM client screening visit, this documentation is equivalent to a quick claim submission.
Review of assessed needs is processed for $10 per client. If referrals are made, it is processed at $17 for that client. Quick claim revenue will show up on the EWM billing authorization.
Documentation Requirements-
- Completion of the GREEN box on the EWM Screening Card below is equivalent to submitting a quick claim for assessment of needs and referral if appropriate.
No Referral Needed – Acknowledgement that clinic staff have discussed potential barriers and client currently denies need for referral.
Community Referral Made – Client referred to appropriate resources that provide food, shelter, help with paying utilities or rent etc. and clinic utilization of Language Line or an outside interpreter during the clinic visit.
Unite Us Network Referral Made – Referral to Unite Us is a partnership that connects clients with resources across the 211 network, a FREE 3-digit helpline for local support and resources.
