FIT Kits Available to Clinics

The Nebraska Colon Cancer Screening Program (NCP) has a surplus of 700 FIT kit tests. We are passing them on to our participating clinics as a pilot project.

Clinics may request either 25 or 50 tests for use with patients 45-74 years of age.

About the Tests:

• FIT test kits expire on 11/11/26: the lab needs to receive the kit before the expiration date
• FIT test kits will be processed by Lincoln Lancaster County Health Department’s laboratory at no charge to the clinic or the patient
• There are no dietary restrictions with these tests

Clinic’s Responsibility:

1. Ensure each patient completes the FIT Kit Enrollment/Request Form, that will be attached to the FIT kit, prior to receiving a FIT kit:
   
   • FIT Kit Enrollment/Request Form (English)
   • FIT Kit Enrollment/Request Form (Spanish) 
2. Match enrollment number and FIT Kit number 
3. Submit patient FIT Kit Enrollment/Request Forms (can be mailed or faxed to NCP at 402-471-0913)
4. Ensure patients meet age requirements: 45-74 years of age (be at least 45 years of age and not have had their 75th birthday)
5. Provide instructions on how to complete the test and the importance of preventive screening
6. Write patient information and date sample collected on the vial and quarter sheet to be mailed back to the lab
7. Provide strong patient education on timely completion of FIT test and returning in the mail within 3 days of completing test

Tracking FIT Test Results:

• Each FIT test has a number on the FIT Kit envelope and on the Request Form.
• Patients must receive the FIT Kit that matches the mailing envelope.
• The Laboratory processes the FIT kits and reports the results to the provider, the patient and NCP.
• Program staff will follow up with provider if results are positive to assist in
  obtaining resources for colonoscopy if needed.
