On-demand Care Coordination Onboarding training is now LIVE on our website!
This training is required for all new care coordinators before rendering services.
Care coordinators must
- watch the entire training,
- take the quiz at the end, and
- get a passing score to get a certificate of completion.
Care coordinators are expected to print or save this certificate for your agency's provider records as proof of training completion.
Training can be found on our Provider Training and Information Sessions webpage under "On-Demand Training Opportunities."

Additionally, the SMART goal slides from this presentation are now available on-demand for care coordinators to review and use while writing member plans of care.
The 1915(i) position is a full-time position with an opportunity to work remotely from anywhere in North Dakota.

This update is to alert you to two new policies with effective dates of November 20.
Providers will now be screened for felony convictions only and those convictions are either considered direct-bearing or non-direct bearing. Each category has a different period of sufficient rehabilitation.
Updates include sections on what to do when transferring or terminating services for supportive services and care coordination. New to this final policy are process flows mapping out steps to take in each situation.

Care Coordination Request Report (CCRR)
This form has been updated to align the "initial contact" language with updated policy, change "individual" to "member" throughout, and remove language stating that 1915(i) staff will help connect a member and the care coordination agency if the agency is having difficulty contacting the member.
The World Health Organization Disability Assessment Schedule (WHODAS) 2.0 is one assessment tool used, in part, to determine a Medicaid member's eligibility for 1915(i).
Who can administer the WHODAS?
Anyone who has completed the following training requirements. Agencies should document staff completion of these requirements in their personnel files.

What is an eligibility redetermination?
Eligibility redetermination is the process of a 1915(i) member who is currently 1915(i) eligible reapplying for 1915(i) to see if they still meet eligibility requirements. Redetermination cases are when a member's 1915(i) application is submitted within 30 days of the member's 1915(i) eligibility expiration date AND there has been no lapse in their eligibility span.
Applications submitted after a member's eligibility span ends are considered a new 1915(i) application, not an eligibility redetermination. When there has been a break in a member's eligibility, a care coordinator must submit a Care Coordination Request Report and cannot bill for time spent helping a member complete their eligibility application.
For eligibility redeterminations, how does a care coordinator get a member's redetermination eligibility documents? (i.e., the member's application, WHODAS/DLA, etc.).
Please email nd1915i@nd.gov and state "Member (name) was redetermined eligible for 1915(i). I would like their new eligibility documents."
