The annual enrollment period for our Health Plan, Dental Plan, Community Eye Care and Flexible Spending Account Plan will begin May 15, 2023 and end June 14, 2023.

HEALTH PLAN
No premium increase has been recommended for the 2023-2024 plan year.**
There are no benefit changes.
If you are making no changes to your health plan coverage, you do not need to submit an enrollment/change form.
If you are making changes to your coverage, the annual enrollment form must be completed, returned and received in the Human Resources office no later than 5 p.m. on Wednesday, June 14, 2023.

During the annual enrollment period, you may:
- Add eligible dependents to the plan
- To enroll a dependent, please complete the annual enrollment form in its entirety, by listing all dependents you wish to cover for the plan year.

Remove dependents, but only if they are enrolled in another qualified health plan
- To remove a dependent from the plan, you must complete the annual enrollment form by listing all the dependents you currently cover, indicating the dependent you wish to remove from coverage.

Change plan option (Traditional to Direct Primary Care OR Direct Primary Care to Traditional)

Spouse Eligibility - Reminder: if your dependent spouse is eligible for health insurance coverage offered through his/her employer, the dependent spouse is not eligible for health insurance coverage under the Union County Employee Group Health Plan.
- Each employee who has elected the Direct Primary Care option will need to complete the Everside Health membership form. These may be submitted to Human Resources.
DENTAL PLAN
No premium increase has been recommended for the 2023-2024 plan year.** There are no benefit changes.
If you are making no changes to your health plan coverage, you do not need to submit an enrollment/change form.
If you wish to enroll or make changes, the enrollment form must be completed, returned and received by Human Resources no later than 5 p.m. on Wednesday, June 14.
Dependents are eligible for our dental plan IF they (1) are covered by our medical plan, OR (2) have medical coverage through their employer, but are not offered a dental plan (spouse only).
There will be a waiting period for coverage of certain services for dependents added during open enrollment. Please see the “Penalty for Late Enrollees” in the Dental Benefits section of the Summary Plan Document on County Connect for more information.
COMMUNITY EYE CARE
If you are making no changes to your coverage, you do not need to submit an enrollment/change form.
If you wish to enroll or make changes, the enrollment form must be completed, returned and received by Human Resources no later than 5 p.m. on Wednesday, June 14, 2023.

FLEXIBLE SPENDING ACCOUNTS
If you wish to participate, you must submit a new Flexible Spending Account form for the 2023-2024 plan year. The annual enrollment form must be completed, returned and received by Human Resources no later than 5 p.m. on Wednesday, June 14, 2023.

**All changes and premium rates are contingent upon the adoption of the FY2024 budget by the Board of County Commissioners. Unless you are notified otherwise, the proposed rates and coverage will become effective July 1, 2023.
