Dear New State of Montana Benefit Plan (State Plan) Member,

Welcome to the State Plan!! As a new State Plan Member, the Health Care & Benefits Division (HCBD) would like to take this opportunity to remind you about two very valuable features of the State Plan.

Live Life Well

 

Live Life Well: State of Montana Wellness Program

The State Plan offers a wide variety of wellness programs designed to help members reach their personal health goals. Please click HERE for a summary of the wellness benefits available to you, or visit our web site at www.benefits.mt.gov/livelifewell.

Live Life Well Incentive Program

Earn $30 per month off your 2021 monthly benefit contribution by completing the required Live Life Well Incentive activities between November 1, 2019 and October 31, 2020. Double your incentive if your spouse or domestic partner covered on the State Plan also completes the activities.

  • Complete a State-sponsored health screening;
  • Self-report you are nicotine free or have completed an alternative activity; and,
  • Self-report you have completed an eligible Next Step activity.

It is recommended that you review your State-sponsored health screening results and choose an eligible Next Step activity related to your results (as applicable).  Please contact our office if you need any assistance.

If you are hired between September 1, 2020 and October 31, 2020 and want to earn the incentive, you will need to enroll in benefits as soon as possible and you must complete all three activities described above. Within a few days of completing your online enrollment, you will be able to schedule a State-sponsored health screening appointment. Please note there is no guarantee an appointment will be available. To self-report activities, visit www.myactivehealth.com/som. If you are unable to set-up an account and self-report your activities (which may happen due to the short time-frame for enrollment information to be shared) contact HCBD right away for an alternative self-reporting method.

The Montana Health Centers offer the same kinds of services you would find at your regular doctor’s office and more, all at no-cost to you and a much lower cost to our self-funded State Plan! Montana Health Centers are available in Anaconda, Billings, Butte, Helena, or Missoula.

Benefits of the Montana Health Centers

  • Full Service Primary Care Clinic - The Montana Health Centers offer the same kinds of services you would find at your regular doctor’s office, plus more! Primary care, acute care, chronic disease management, blood work and labs, health coaching services, medication refills, X-ray and radiology referrals, and routine and wellness visits.
  • Lower Healthcare Costs - No deductibles or copays for any service received at a Montana Health Center. A visit to a Montana Health Center costs you and the State Plan less than the same type of visit at a regular doctor’s office!
  • Minimal Wait Times - Average time spent in the waiting room is only 5 minutes!
