Dear Rural Health Partner:
The Department of Public Health and Human Services (DPHHS), as a part of the Rural Health Transformation Program (RHTP), is accepting funding proposals from emergency medical services (EMS) agencies to administer Community Integrated Health (CIH) programs.
Montana’s RHTP plan addresses the root causes of rural health disparities through five integrated initiatives. Initiative Three, Innovative Care Models, supports modernizing care delivery through several sub-initiatives, including funding CIH pilot sites that provide services through infrastructure investments, supplies, and employees.
This grant opportunity is open to EMS agencies located in or serving rural areas of Montana. Applicants must demonstrate their ability to administer the program and obtain clinical oversight from a medical director.
Priority consideration will be given to agencies that:
- Currently operate a CIH program in a rural or frontier area.

Need additional funding to sustain operations or expand services.
The online portal to submit funding proposals will be available through Sept. 15, 2026. Apply via Submittable. It is entitled “FY26 DPHHS Community Integrated Health Pilot Site Grant Application.”
The Vendor Information page at ruralhealth.mt.gov includes resources about using Submittable. DPHHS expects to announce additional grant opportunities in the coming months and will post them on that page as they become available.
