To all interested parties,
A major focus of the Montana Department of Health and Human Services' (DPHHS) Rural Health Transformation Program (RHTP) is to expand access to care to the state's rural Medicaid/Medicare dual-eligible population. This may include the elderly and individuals with eligible disabilities — developmentally disabled program (DDP), severely disabling mental illness (SDMI), social security disability insurance (SSDI), individuals in need of long-term services and support (LTSS), and/or with complex chronic conditions.
DPHHS has released a Request for Information (RFI) to solicit recommendations for integrated care and payment models for dual-eligible individuals in rural Montana. Findings from this RFI will inform the design of a subsequent Request for Proposal (RFP) for pilot programs aimed at improving health outcomes for this population.
This notice enables stakeholders to provide feedback, ensuring that this RFI accurately reflects the community's needs.
The RFI, released through eMACS and will be open to response for 30 days following the release.
