When performing a change household ID, an aide can only complete the first three steps. A CPA must complete the “adjust benefits” screen. If an aide attempts to complete the adjust benefits screen, benefits will remain with the original household and not transfer to the foster household. |

The deadline to complete retailer monitoring is fast approaching, please make sure all visits are completed on time. Often, issues such as stocking violations are identified during monitoring that require follow up with the store, and sometimes even a second in-person visit. This can take time, so please do not leave monitoring until the last minute because even if a monitoring visit initially takes place before the deadline, if there is follow up that must occur after the deadline it will not be considered a complete monitoring. Please reach out to Lori with any questions about the monitoring process. |

Defined as: History of diagnosed Gestational Diabetes Mellitus (GDM).
Presence of condition diagnosed, documented, or reported by a physician or someone working under a physician’s orders, or as self-reported by applicant/participant.
*Self-reporting of a diagnosis by a medical professional should not be confused with self-diagnosis, where a person simply claims to have had a medical condition without any reference to professional diagnosis.

- Educate on appropriate weight gain for pregnancy.
- Encourage regular prenatal care throughout the pregnancy.
- Encourage them to discuss physical activity recommendations with their healthcare provider.
- Educate on maintaining a healthy diet, including prenatal vitamins.
Postpartum women with a history of GDM:
- Counsel them on achieving and maintaining a healthy weight, physical activity, and following a healthy diet.
- Encourage them to attend all health care appointments, including the 4-6-week postpartum visit.
- Provide breastfeeding support and refer to the breastfeeding expert.
- Encourage women to discuss their breastfeeding goals with HCP, especially if medications are prescribed.
