Asthma Logo 

Diagnosing and Managing Asthma (DMA Program) 

Funding award: $3,000 plus scholarships to the Big Sky Pulmonary Conference and annual Association of Asthma Educators Review course

Health Systems / Primary Care / Outpatient Care: The DMA program works to enhance workflows around the care of patients with asthma. This improves guidelines-based care and improves access to quality asthma care and self-management education.

 

Asthma Hospital Patient Education, Action Plan, and Discharge Protocol (AHEAD Program)

Funding award: $3,000 plus scholarships to the Big Sky Pulmonary Conference and annual Association of Asthma Educators Review course 

Emergency Departments: this program improves workflows to improve patient education, encourage outpatient care, and works to reduce readmissions from asthma exacerbations.

 Diabetes Program Logo

Electronic Continuous Quality Improvement  (eCQI Program)

Funding award: $5,000 

Health Systems / Primary Care / Outpatient Care: Develop a project focused on utilizing electronic continuous quality improvement (eCQI) to improve workflows for people with diabetes or prediabetes and generate referrals to diabetes prevention programs (DPP) or diabetes self-management education and support programs (DSMES). 

 

Chronic Kidney Disease (CKD) Screening and Early Detection

Funding award: $2,000

Health Systems / Primary Care / Outpatient Care: Identify and work to improve early detection and screening for CKD in patients with diabetes utilizing eCQI and clinical decision support tools. 

 

Retinopathy Screening

Funding award: $2,000

Health Systems / Primary Care / Outpatient Care: Identify and work on a retinopathy related project utilizing eCQI and clinical decision support tools to increase the number of screenings completed. Sites may also be awarded retinopathy cameras, valued at $5,200 each, based upon need.

Other Opportunities From the Diabetes Program

 

New DPP Site

Funding award: $13,000

Health Systems / Primary Care / Local Public Health: Develop a plan to implement the National Diabetes Prevention Program in person or via telehealth, including administrative support, provider referrals, marketing and outreach, lifestyle coach staffing and training, participant recruitment, and delivery of the curriculum in under served areas or to target populations.

 

DPP Recruitment and Retention

Funding award: $5,000

Current DPP sites: Complete a project aimed at increasing recruitment and retention among an under served population. Sites will focus on projects aimed at increasing retention within programs, which may include working with CONNECT, social determinants of health, increasing access or addressing specific populations. 

 

DPP Enrollment

Funding award: $2,000

Current DPP sites: Complete a project aimed at increasing enrollment. Projects can be focused around addressing social determinants of health, working with a local food bank, WIC or a priority population to increase enrollment.

 

DSMES Accreditation/Recognition or Quality Improvement

Funding award: $5,000

Health Systems / Primary Care / Outpatient Care / Pharmacy: Programs will receive funding to work toward recognition/accreditation for onsite or telehealth services. For those sites who have achieved recognition/accreditation funding can be utilized to work on a quality improvement project focused on increasing enrollment and referrals to DSMES. 

 

DSMES Telehealth

Funding award: $5,000

Health Systems / Primary Care / Outpatient Care: Programs will receive funding to work toward establishing or expanding telehealth within their DSMES programs.
