The City of Minneapolis Office of Violence Prevention has recently released a public survey. The results will be used to guide the City of Minneapolis Health Department's Office of Violence Prevention's first strategic plan.

You, as a part of the Minneapolis community, know best about pressing issues. Your opinion is valuable to our community.

Your privacy is important to us: What you provide in this survey may be shared with Minneapolis Health Department employees. You are not legally required to take this survey. You will not be asked for your name, but we ask of some information about your race, age, and neighborhood. Need Feedback on Program Finder

The Youth Coordinating Board has heard from youth, families and providers in Minneapolis that it's difficult to find information in a convenient way about all the afterschool programs around us. When asked, the number one reason why youth don't participate in afterschool activities is because they don't know what's out there. We're hoping to fix this issue! However, we're need to gather some information as we take on this project. It would help us greatly if you could please take this short 6-question survey and share it with those in your network.
