Vaccination Event Information & Consent Forms

Since you are eligible to receive the COVID-19 vaccine we wanted to remind you of how easy the State of Missouri is making it for you to receive it! You will receive this email on a weekly basis and it will provide updated event information for your convenience. There are two channels for you to receive the vaccine:

Missouri Disaster Medical Assistance Team (DMAT) events across the state: For all State of Missouri team members
- Schedule an appointment here - State of Missouri Employee Registration. Please note that when signing up you need to select the ellipses(...) next to your preferred time. Hovering over or clicking on the time will not work. Click “Edit Item” to enter your personal information. You will not receive a confirmation e-mail, but you can confirm your registration by clicking on the Confirmed Registration link and searching for your agency and last 4 digits of your phone number that you provided on your registration form.
- You will be required to present your driver’s license at registration. We ask for an email address during the registration process in case we need to make contact if there is a change in the schedule or cancellation. In addition, by providing an email it will also allow us to send a reminder of your second appointment as that date approaches.
- NOTE: You need to complete two consent forms (attached) and bring them with you to the appointment.

Dependent upon vaccine availability, daily appointments at the Strive for Wellness Clinic (Truman building room 478 in Jefferson City) for active state employee subscribers who are enrolled in an MCHCP medical plan through Anthem.
- Make an appointment by logging into your account here: https://my.mchcp.org/login.aspx. Instructions are attached in the MCHCP CovidVaccinationAppointmentFlier.
- Six parking spots in Lot 7 on McCarty Street have been reserved for your use while you are in the Truman building receiving your vaccination. MCHCP COVID Parking Map is attached.
- Remember to print out and then complete your SFW HealthCenterCOVID-19 Vaccination Forms (available on the myMCHCP website) and bring it with you at the time of your appointment.
The Vaccine Navigator is another option you can use for registering for COVID-19 vaccinations. Registration within Vaccine Navigator can be completed at https://covidvaccine.mo.gov/navigator/. Once registered within the Vaccine Navigator system, you will receive a scheduling link when your assigned phase and tier is open, and there is an event within your designated region. Events posted within this system are usually public events. Open events within each region can be found at https://covidvaccine.mo.gov/events/.
Remember:
- Appointments are required to receive a vaccine.
- Masks and social distancing are required at all clinics.
- Do not show up to a vaccine clinic if you are sick.
- This is a dynamic situation, so check back often for new availability of appointments
If you are considered to be at-risk (over the age of 65 or with a serious health condition), please do not wait for a state-sponsored event. If you have not already done so, you should contact your healthcare provider or local public health agency to begin the vaccination process.
