Rural Health Transformation Program Expands Investment in Home Visiting Services

JEFFERSON CITY, Mo.The Missouri Department of Social Services’ Rural Health Transformation Program (RHTP) is investing in expanded nurse-based home visiting services across rural Missouri. Through this investment, the Missouri Children’s Trust Fund (CTF) has released a Request for Proposals (RFP) for eligible organizations to implement or expand evidence-based home visiting programs. 

Through RHTP, a total of $588,000 available annually to support the implementation and expansion of evidence-based, Home Visiting Evidence of Effectiveness (HomVEE)-approved nurse home visiting programs in rural communities. Funding includes up to $100,000 for model fees and training for new nurse-based home visitors and up to $488,000 to expand existing services.

This investment is designed to strengthen families, improve maternal and child health outcomes, and help prevent child abuse and neglect by increasing access to high-quality home visiting services in rural communities.

The Missouri CTF will administer the funding opportunity and oversee the application process on behalf of the RHTP.

Eligibility:

Eligible applicants must currently operate, or be actively working to implement, an evidence-based HomVEE-approved home visiting model, demonstrate fidelity to the model, and provide services delivered by nurses. Proposed services must expand existing programming and may not replace current funding.

Timing and Next Steps:

Organizations interested in applying are encouraged to attend a virtual informational meeting on July 8, 2026, from 9 to 10 a.m. and can register here.

Applications must be submitted by the end of the day on August 23, 2026, via email to Melinda.Kirsch@oa.mo.gov.
