Show-Me Home Program Helps 2,500 Missourians Transition to Community Living

JEFFERSON CITY, MO - The Missouri Department of Social Services (DSS) announced today that the Show-Me Home program has helped 2,500 Missourians move from facility settings back into the community since the program’s inception in 2007.

Show-Me Home is the name of Missouri’s Money Follows the Person program, which is a federally funded program aimed at helping older adults or physically disabled adults to integrate back to living independently. The program aims to assist Medicaid-eligible individuals living in nursing home settings who wish to live independently, with the proper services and supports.

"With the right mix of home and community-based services, along with the case management and assistance the program has provided since 2007, we are honored to know 2,500 Missourians have had the opportunity to reintegrate into their communities," said Shawn Brice, Director of the Show-Me Home program at MO HealthNet. "The program is a collaboration with our partners at the Department of Health and Senior Services, and the many organizations across the state who work with us to advocate for people so that they can live independently, and who may not otherwise have been able to realize that goal."

The Show-Me Home program is thankful for support received at the federal, state, and local levels to reach this achievement, and will continue to help nursing home residents navigate a variety of complex systems and barriers to realize their dream of living independently again.

To meet the criteria for eligibility to participate in the Show-Me Home program, a person must:

  • Currently reside in a nursing facility for at least 60 days
  • Have a physical disability or be at least 63 years old
  • Be eligible for, or a current participant in, Medicaid (MO HealthNet) before moving out of a nursing facility
  • Move into a home/apartment that is leased or owned by the individual or a family member
