Petroleum Remediation Program Reporting Form Updates

The Petroleum Remediation Program has posted updates to the Investigation, Monitoring, and Corrective Action guidance document report forms (https://www.pca.state.mn.us/waste/cleanup-guidance). The forms are now unlocked and allow for highlighting and tracking changes during internal reviews prior to submitting to the MPCA. Earlier versions posted just before Consultants’ Day were locked. With the unlocked versions, do not revise or delete any of the questions within these forms. Please start using the updated forms now, as they will be required by May 1, 2021.

The update affects the following:

4-06 – Investigation report

4-08 – Monitoring report

7-02 – Conceptual corrective action design (CCAD) report

7-03 – Focused investigation work plan

7-04 – Focused investigation report

7-05 – Pilot test work plan

7-06 – Pilot test report

7-07a – Remediation system detailed corrective action design (SDCAD) report

7-07b – Excavation detailed corrective action design (EDCAD) report

7-08 – Remediation system operation monitoring (RSOM) report

The air emissions screening spreadsheet for remediation systems has been removed and replaced with the air emissions Risk assessment screening spreadsheet. For any system reporting needs, please use this spreadsheet effective immediately. Guidance document 7-06 Pilot test report, and 7-08 Remediation system operation monitoring (RSOM) report have been updated to reference this spreadsheet.
