 General
industrial stormwater permittees:
It’s time to fulfill an annual industrial stormwater permit
requirement by filling out your facility’s Annual Report. The 2017 general industrial stormwater annual
report form is online and due March 31,
2018.
Permittees can either fill out the Microsoft
Word form or the PDF form.
Enter your Permit ID number plus information / data for all months your
facility was active during 2017; for most Permittees, that’s the entire 2017
calendar year.
If you fill out the form electronically, email it to the Industrial
Stormwater Program. You have the option
to mail it in, if you prefer.
We are also mailing out copies of the newsletter & instructions to
every Permittee “site contact.” You can also read the Annual
Report newsletter that contains instructions on filling
out the annual report as well as an announcement of 2018 industrial stormwater
training opportunities.
Individual wastewater
permittees with a stormwater chapter:
For permittees with an individual wastewater permit with a
stormwater chapter, part of your requirements include filling out an industrial
stormwater annual report. This permit
requirement is for permittees only; it is not required for those who have
certified for No Exposure. If you are not required to fill out an industrial
stormwater annual report, please delete this message.
There is a specific (and shorter!) version of the Industrial
Stormwater Annual Report form just for you. Please fill out either the
Microsoft Word form or PDF form.

Due date: Please read your permit language for your plant’s
annual report due date; the date varies by facility.
