Public comment closes Sept. 26
Congress recently established a $50 billion Rural Health Transformation Program to help rural communities reimagine their health care delivery systems and improve health outcomes.
This program aims to address health care challenges facing rural communities. Funding will focus on promoting innovation, strategic partnerships, infrastructure development, and workforce investment. The goal is to support rural population health care initiatives, promote preventative health, and address root causes of disease.
To put this funding to good use, MDH is seeking input and ideas on how to transform rural health and strengthen rural health care delivery. This includes ideas related to emergency services, primary care, care for people with complex and chronic conditions, mental and behavioral health care, maternal health care, and other at-risk services.

All voices and perspectives are welcomed to help MDH identify rural needs and opportunities. This includes:
- Health care providers in all settings
- Administrators or workers in all types of hospitals (small independent, critical access, etc.)
- Community groups and/or advocacy organizations
